ACS Registration Requirements & Forms

Dear Prospective ACS Parents,

We are pleased that you are considering the American Cooperative School for your child's education! For each child you wish to enroll, please submit one each of the following documents to ACS.

Application Documents, Records and Testing
(Please read the application document submission notes at the bottom of this page before proceeding.)

1. ACS Registration Form:

2. Medical Information Form:

3. Immunization Form:

4. Authorization for Release of School Records:

5. Teacher's Letter of Recommendation:

One letter is required for applicants for Grades 1 - 5
Two letters are required for applicants for Grades 6 - 12

6. Counselor's or Principal's Letter of Recommendation:

One letter is required for applicants for Grades 1 - 5
One letter is required for applicants for Grades 6 - 12

7. Report cards or transcripts from previous school

Over the past year for applicants for Grades KG1 - 2
Over the past three years for applicants for Grades 3 - 12

8. Results of standardized testing, if available (ITBS or CTBS)

9. Special education documentation (if applicable)

10. Copy of birth certificate or passport (for all grades)

11. Testing: All elementary school applicants will be scheduled for English proficiency testing. Testing in Math, and in some cases English or Spanish, is required for secondary school applicants.

Registration Process
Enrollment decisions will be made by the appropriate principal after all documents, records and test results are reviewed. Parents are strongly encouraged to meet with their child's future principal.

Payment of Fees
There is an application fee of 100 dollars (US) for each child who applies for entrance into ACS. If accepted, ACS will credit the application fee toward the child's tuition fees. There is also a one-time capital fee that must be paid by all new students. Upon acceptance into ACS, the capital fee and tuition fees must be paid in the ACS Business Office before a child may attend class.

Document File Formats
For the numbered items 1 - 6 above, CHOOSE only one file format (pdf or rtf) for your application documents. Make this choice based on how you will apply to ACS.

-Files that end in ".pdf" require may require Adobe Acrobat Reader to open.
-Files that end in ".rtf" require Word Pad, MS Word, AppleWorks, etc. to open.
-Rtf files can be easily edited electronically (and therefore, when complete, submitted electronically). Both pdf's and rtf's may be printed, filled out by hand, and sent via snail mail.
